Advance care planning involves choosing decision makers and preparing them to make decisions for you if you're unable to communicate. This process should begin with a conversation when you're healthy, before a crisis occurs. It's important for everyone to plan for future medical decision making. By understanding advance care planning, your rights as a patient, and the decisions you should consider, you can develop a strong plan.
Resources are available to help you with this process. This session is designed for individuals interested in completing their own advance care plan and is free, but advance registration is required.
