Advance care planning is the process of thinking about and discussing your wishes for future medical care, especially in situations where you may not be able to make decisions for yourself. This can involve talking to loved ones about your values and preferences, and it often includes creating legal documents called durable do not resuscitates, medical power of attorneys, living wills, and advance directives.
