Advance Care Planning

Make your wishes known. Give your loved ones clarity.

Planning ahead starts with understanding what matters to you. We help you explore your healthcare wishes, organize them in an advance medical directive, and prepare to share them with the people you trust.

We’ll Help You:

  • Put your healthcare wishes into writing.

  • Choose someone you trust to speak for you if you can’t.

  • Prepare to talk with your loved ones about what matters to you.

  • Keep your documents organized and easy to find and share.

What Is an Advance Care Directive?

An advance care directive records your healthcare wishes and identifies someone to make healthcare decisions if you cannot speak for yourself. We help you work through the questions and document your choices in plain language.

You don’t need to be seriously ill to get started. We support people planning ahead, living with illness, or updating an existing plan.

What to Expect

1. Start with a conversation.
Tell us what you need help with and ask your questions. There is no charge for this initial conversation.

2. Talk through your wishes.
During your planning appointment, we explore what matters to you, discuss your preferences, and help you identify your healthcare decision-maker.

3. Receive your document.
We prepare your typed directive for you to review and provide guidance on signing and sharing it. You’ll arrange any required           witnesses.
