Care Navigation & Caregiver Support

You don’t have to figure out every next step alone.

Managing appointments, finding resources, and supporting someone you love can feel overwhelming. We help you organize the details, prepare your questions, and make a practical plan for what comes next.

We’ll Help You:

  • Organize appointments, important information, and next steps.

  • Prepare questions for your healthcare team.

  • Find community services and support options.

  • Plan how family members can share caregiving responsibilities.

  • Feel more supported during healthcare appointments.

What Does Care Navigation Mean?

Care navigation means having someone help you sort through the practical details of getting support. You may need help finding resources, keeping track of information, or deciding whom to contact next.

We help you get organized and prepare for conversations with the professionals involved in your care.

Support for Caregivers

When you’re caring for someone else, your own needs can get pushed aside. We provide a listening ear and help you identify practical ways to make your responsibilities more manageable.

Together, we can organize priorities, explore support options, and prepare to ask family members or community services for help.

Support at Appointments

We can meet you at healthcare appointments to help you:

  • Organize the questions you want to ask.

  • Take notes so you can review the conversation afterward.

  • Speak up about your concerns and preferences.

  • Keep track of instructions and follow-up tasks.

You’ll arrange your own transportation. We meet you at the appointment location.

What to Expect

1. Tell us what’s going on.

We start with a conversation about your situation and where you need support. There is no charge for this initial conversation.

2. Choose your priorities.

Together, we identify the tasks you want help with and agree on the support, estimated time, and cost before paid work begins.

3. Work through the next steps.

We provide the agreed support and check in as your needs change. You stay involved in decisions about your care and your family.
